Getting started

OCIVO is a phone- and tablet-based vision screen that takes 8–10 minutes and tells each person what to do next. It is a DoktorConnect engine, sister to Otivo (hearing). It screens; it does not diagnose. All pass lines are provisional until the consultant ophthalmologists sign them off.

Who does what

Role Can do Cannot do
Person being screened Take a screen, see their result, keep screening at home with a patient code See anyone else's data
Clinic staff Register patients, run screens, manage follow-ups, share results See other clinics' patients
Clinic admin Everything staff can, plus approve staff, choose checks, import patients Remove the clinic's last admin
Eye specialist Enter chart acuity, record outcomes and reference exams Change a saved screen (results are locked)
Platform admin Add clinics, switch clinics off, grant research access Read any patient or screen (counts only)
Researcher See de-identified, research-consented screens and statistics See names, codes, phones or birth years

Signing in. Every signed-in role uses the same email sign-in. Enter your email, open the link OCIVO sends, and open it in the same browser. A person screening themselves can skip sign-in, but their result is then not saved.

The app's tabs. Person's journey runs a screen. Journey map shows every step and the design notes. Clinic is for staff. Eye specialist is the referral inbox. My results appears when a patient is signed in. Design notes and Specialist mode sit at the top right.

Running a clinic screen

A clinic screen takes 8–10 minutes and follows the same steps every time; OCIVO skips any step a person does not need. The order is: register the patient, safety check, questions, set up the screen, distance test (right eye, then left), pinhole (only if an eye fails), near test, extra checks, blood pressure and sugar, result, next step and save. The clock starts at the first tap and stops at the result.

What you need: the phone or tablet at full brightness, a bank card for calibration, a tape measure or a 3 m mark on the floor, a pinhole card, and red-cyan 3D glasses for children.

  1. Register the patient. In the Clinic tab, enter a clinic file number (not a name) and year of birth. OCIVO creates a 6-character patient code. Tap Continue.
  2. Consent. Read the statement to the patient and tick I understand. The research box is optional; screening does not depend on it.
  3. Safety check. Ask each question. Any yes (sudden vision change, a curtain or shadow, painful red eye, new double vision, eye injury, stroke signs, a white glow in the pupil) stops the screen and sends the person for same-day care.
  4. Questions. Age, glasses, diabetes, blood pressure, family glaucoma and the rest. These build the Eye-Health Risk Profile. Under 12, the children's questions appear instead (see Screening children).
  5. Set up the screen. Hold a bank card against the screen and slide until the outline matches. OCIVO remembers this on the device for 7 days. Choose the distance (3 m standard, 2 m in a small room) and who answers (a helper taps, or the patient speaks or taps).
  6. Distance test. The patient covers the left eye with a palm, both eyes open, no pressing. One E appears; they say or show which way its bars point, and the helper taps the arrow. Can't tell is always allowed. Then the right eye is covered and the left tested. About a minute per eye.
  7. Pinhole. Only if an eye is below the line: the patient looks through a pinhole card. If vision improves, glasses will likely help; if not, it is a doctor referral.
  8. Near test. At reading distance (about 40 cm), both eyes. Skipped under age 8.
  9. Extra checks. Whichever the clinic has switched on and the age allows: macular grid, contrast, colour, and the 3D check for children.
  10. Blood pressure and sugar (adults). Type the readings, or tap Get latest readings if the patient is linked to DoktorConnect. Skip if not measured.
  11. Result. The result, what it means and the AI-written report. See Results, the AI report and sharing.
  12. Next step. Enter the patient's mobile number and tick reminders consent if they agree, then Finish and save. Follow-ups open automatically.

Extra checks at a glance

Check Ages How What a problem means
Macular grid 12+ Each eye at 30 cm; tap any wavy or missing squares See an eye doctor
Contrast vision 12+ Pale E at the test distance Gap of 4 lines or more: see an eye doctor
Colour vision 5+ 8 plates: tap where the ring's gap is Blue-yellow loss adds risk; red-green difference is noted only (research module)
3D vision 3–11 Red-cyan glasses, 40 cm, find the hidden shape Not passed: see an eye doctor

Camera checks. If the clinic has them on, a chip in the test bar warns Move closer or Is your right eye covered?. They are advice only, and the camera picture never leaves the phone.

Screening children

OCIVO screens from age 3. Enter the child's age on the questions screen and the children's pathway switches on by itself. Under age 3, OCIVO asks for a photoscreener or an eye professional instead.

Before you start: print the HOTV matching card (a link on the test screen opens it), have an eye patch or tape and tissue, and red-cyan paper 3D glasses. Ages 3 and 4 need two adults: a parent sits with the child and a second adult taps the answers.

  1. Children's questions (under 12): glasses, last eye exam, an eye turn the parent has noticed, squinting or sitting close to screens, born very early or very small, lazy eye or eye turn in the family, diabetes, sickle cell.
  2. Letters instead of the E (ages 3–7): one letter at a time, H, O, T or V. The child points to the same letter on the matching card; the helper taps it. The child does not need to read.
  3. Distance: 1.5 m for ages 3–5, 3 m from age 6. OCIVO sets this for you.
  4. Cover the eye properly: use a patch or tape and tissue, never a hand. Children peek.
  5. The 3D check (ages 3–11): take the patch off, put the glasses on with the red lens over the left eye (over their own glasses if they wear them), and hold the screen 40 cm away. The first picture is practice. Then the child points to the hidden shape; the pictures get harder until they stop seeing it. Without the glasses nothing can be seen.

What changes by age

Age Letters Distance Pinhole and near Extra checks
3–4 HOTV 1.5 m Skipped 3D
5 HOTV 1.5 m Skipped 3D, colour
6–7 HOTV 3 m Skipped 3D, colour
8–11 Tumbling E 3 m Done 3D, colour
12–17 Tumbling E 3 m Done Macular, contrast, colour

When a child is referred. Below the vision line for their age, a gap of two lines or more between the eyes, an eye turn the parent has noticed, or not passing the 3D check (400 seconds of arc under age 5, 200 from age 5, provisional) all send the child to an eye doctor. Born very early with no eye exam adds a full eye examination. The report is written to the parent.

Results, the AI report and sharing

Every screen ends with one clear next step, shown as a coloured banner, then a plain-language report written by AI and checked before it is shown.

Banner Meaning Follow-up due in
Urgent: get eye care today A red flag or severe loss 1 day
See a doctor today about your blood pressure or sugar BP 180/120 or higher, or very low sugar 1 day
See an eye doctor Loss not fixed by pinhole, eye difference, macular or 3D problem 28 days
See a doctor this week about your blood pressure or sugar BP 160/100 or higher 7 days
Get your eyes checked for glasses Loss that the pinhole improves 90 days
Test again The screen was not reliable —
See a doctor about your blood pressure or sugar BP 140/90 or higher, or sugar in the diabetes range 28 days
Book an eye health check Vision fine but risk factors (for example diabetes, family glaucoma) 90 days
Screen passed All lines met, no risk rules triggered —

All thresholds are provisional until the consultants sign them off.

Two scores, kept apart. The Vision Function Index says how well the eyes see today. The Eye-Health Risk Profile says how likely hidden eye disease is. A person can see well and still be at risk.

The AI report never names a diagnosis, never says the eyes are healthy, and always says it is a screen. If the AI is unavailable, standard wording is used instead. The clinician addendum (trial detail, calibration, camera readings) is for staff only.

Print. On the result screen, Print or save as PDF before you finish.

The private patient link. After Finish and save, the Screen saved page offers Create patient link. The same button is in the patient's history, the Eye specialist inbox and the patient's own My results.

  1. Tap Create patient link.
  2. Send it one of four ways: Copy it, Share… through the phone (WhatsApp and others), Send text to the number shown, or Send email (or open my email app).
  3. The patient opens a page with their result, what it means, the next step, follow-up dates and the checks done.

The link lasts 30 days, can be withdrawn, and records how often it is opened. It never shows history answers, staff names, patient codes or clinic notes. Anyone holding the link can see the result, so send it only to the patient or, for a child, the parent. Texts need SMS keys on DoktorConnect Core; until then, copy or share it.

Follow-ups and reminders

Every screen that needs action opens a follow-up automatically when it is saved, with a due date set by the result (see the table in the previous section). Staff work them from the Follow-ups panel in the Clinic tab.

The five lists

List Shows
Overdue Past the due date and not yet booked
Due this week Due in the next 7 days
Open Every follow-up not yet booked
Booked An appointment date has been entered
Closed Attended, did not attend, or closed by staff

Working a follow-up

  1. When the patient books, tap Mark booked and enter the appointment date.
  2. After the visit, tap Attended and choose the outcome (for example glasses prescribed, or a medical visit outcome).
  3. If they did not go, tap Did not attend. Close ends it for another reason; Reopen brings it back.
  4. Send reminder now texts the patient straight away. Every attempt is logged on the follow-up.

The call list shows overdue patients whose reminders have not been delivered. Phone these patients yourself.

Automatic reminders go out once a day at 08:00 UTC, only to patients who ticked reminders consent and gave a mobile number.

When Message
Straight away Urgent and same-day results: one message only
Day 3 First reminder to book
Day 10 Second reminder
Every week while overdue Up to 5 more reminders
The day before a booked visit Appointment reminder

Messages carry no clinical detail beyond the next step. They are sent through DoktorConnect Core, so they start reaching phones once the Africa's Talking (Nigeria) and Twilio (US) keys are added there; until then each attempt is logged as not sent and the patient appears on the call list.

Patients at home

A patient screened at a clinic can keep checking their eyes at home, and those home screens join the same record, so the clinic sees one continuous history.

Getting started at home

  1. The clinic gives the patient their 6-character patient code, on a printed slip or read from the screen.
  2. At home, the patient opens myocivo.com and taps I have a patient code.
  3. They sign in with their email and open the link OCIVO sends, in the same browser.
  4. They enter the code. Their clinic screens now appear in My results.

From then on, each home screen runs the same steps as at the clinic, with the checks the clinic has chosen, and is marked Home on the record.

My results lists every screen with its result. When there are at least two screens 90 days or more apart, it shows the trend. A change of 0.15 logMAR or more (about one and a half lines) is flagged. My results also has the patient's DoktorConnect box and Share my latest result.

When a home screen needs action, it appears in the red Home alerts box on the clinic's Clinic tab. Staff open it, contact the patient, and tap Mark reviewed. Home screens are not counted in the staff consistency table.

Helping someone to act. After a result that needs follow-up, OCIVO can offer a short Ready Mindset profile. It finds what is most likely to stop the person from going (cost, fear, time and others), and the AI writes a plan and a first step around it.

Household testing

One account can screen the whole family, the same way Otivo does. Each family member gets their own record at the account holder's clinic, so their results, follow-ups and reminders stay separate.

Adding a family member

  1. Sign in at home and link your own record with your clinic code first.
  2. On the Who are you testing? screen, or in My results, tap Add a family member.
  3. Enter a first name or a label such as Mum, how they are related (child, parent, partner, brother or sister, grandparent, other) and their year of birth. The clinic sees this label too.
  4. Tap Add to my family. OCIVO shows their own patient code.

Testing them

Inviting an adult to test on their own phone

For adults and children aged 12 and over, the added screen offers an invite: Copy, Share…, WhatsApp, Send text or Send email. The invite link (myocivo.com/?invite=XXXXXX) carries only their code, never results. When they open it, OCIVO goes straight to sign-in with the code filled in, and their own account links to the same record.

Anyone with a code can see that person's results, so send an invite only to the person it is for.

Rule Detail
Family size Up to 12 family members per household
A child's reminders Go to the parent's number, with the parent's consent
Under age 3 Can be added, but are screened from age 3; see a photoscreener or eye professional before then
Removing someone Takes them off your list only; the clinic keeps their record

Eye specialist inbox and reference exams

The Eye specialist tab lists every referral from the clinic, so the specialist can record what they found. Those findings also feed the validation study.

Specialist mode (top right) adds a box during a screen for the acuity measured on a standard chart, in 6/12, 20/40 or logMAR. OCIVO compares it with its own estimate.

For each referral in the inbox

  1. Read the summary: result, acuity each eye, pinhole, extra checks and the reason for referral.
  2. Record outcome: what was found and done.
  3. Add reference exam (for the study): examiner, exam date, visual acuity each eye, whether it is presenting or best-corrected, whether the person needs eye care, and the main finding (normal, refractive error, cataract, diabetic retinopathy, glaucoma, macular degeneration, amblyopia, strabismus, other).
  4. Create patient link, if the patient needs their result.

To add a reference exam for a screen that was not referred (the study needs passes too), open the patient from the Clinic tab. The reference exam form is under their latest screen.

Good practice for the study: do the reference exam without looking at the OCIVO result first, and within a short, fixed number of days of the screen. The research console records the gap.

Export pairs downloads OCIVO-against-chart pairs for quick checks. The research console holds the full analysis.

Clinic tab for staff and clinic admins

The Clinic tab is the clinic's workspace. Every screen saved here is signed with the staff member who ran it, and saved results cannot be changed.

Joining a clinic

  1. Sign in with your email in the Clinic tab and enter your name as it should appear on reports.
  2. Enter the clinic's join code (ask your clinic admin).
  3. You see Waiting for approval. You have no access to any patient until an admin approves you.
  4. Once approved, sign in again and the clinic opens.

A new clinic can also be set up from this screen, or by a platform admin in the admin console. Either way, the person who sets it up becomes its first admin.

What staff see

Panel Use
Register a patient File number and year of birth; creates the patient code
Patients Click a code for the full history, latest screen, patient link, reference exam and DoktorConnect box
Follow-ups The five lists, call list and reminders (see Follow-ups and reminders)
Home alerts Home screens that need action
Staff consistency Per staff member: inconclusive rate, median screen time, check letters missed per eye, precision

Clinic admins also see

Panel Use
Staff waiting for approval Approve as staff, Approve as admin or Decline. Approve only people you know work at the clinic
Staff list Deactivate or reactivate staff. A clinic always keeps at least one admin
Checks Switch the macular grid, contrast, colour and 3D checks on or off. The choice also applies to patients' home screens
Camera checks Advisory or off
Camera bench study Measure how well the camera judges distance and a covered eye on your own phones
Admin console, Research console Buttons that open them in a new tab

The camera bench study takes about 15 minutes per phone. First calibrate at 40 cm. Then follow the 18 steps (6 distances, each with no cover, right eye covered and left eye covered). The results table shows the distance error and how often a covered eye was spotted. Save it to the clinic record, or download it as CSV. Camera checks stay advice-only until these results are good enough.

Admin console (/admin)

The admin console at myocivo.com/admin/ is where clinics are onboarded and staffed. Platform admins see counts only, never patients. Clinic admins see only their own clinics. Sign in with the same email as the app.

The clinic directory shows, for each clinic: status, active staff, pending staff requests, patients, screens (all time and last 30 days), open and overdue follow-ups, and the last screen date.

Adding a clinic (platform admins)

  1. Ask the clinic's lead to sign in once at myocivo.com, so their account exists.
  2. Open Add a clinic: clinic name, country, the lead's email and their name on reports.
  3. Add clinic. The lead becomes the clinic's admin, and the join code is shown. You do not become a member, so you still cannot see its patients.
  4. The clinic admin then adds their own staff, or shares the join code and approves the requests.

Managing a clinic (tap Manage)

Research access (platform admins): enter a researcher's email and choose all clinics or one clinic. Remove access with one click.

Importing patients from a spreadsheet (clinic admins)

  1. Download template, or save your spreadsheet from Excel as CSV with these columns:
Column Required Example
ref Yes OPD-1001 (a file number, never a name)
birth_year Yes 2016
consent Yes yes (the patient or a parent agreed to screening)
phone No +2348030000000
reminders No yes / no
research No yes / no
  1. Choose the file. OCIVO shows how many rows it read and how many have consent.
  2. Import. Rows without consent, or with a missing or invalid year of birth, are skipped and listed.
  3. Print code slips: one slip per patient, with only the file number, the code and how to use it at home. No results appear on a slip.
  4. Download codes (clinic file) if you need the list. It identifies patients, so keep it with clinic records.

The first platform admin is added once, directly in Supabase. After that, everything is done in the console.

Research console (/research)

The research console at myocivo.com/research/ holds the validation study. It shows only screens where the patient ticked the research box, and only in de-identified form.

What a researcher sees: participant pseudonyms (P-XXXXXX), staff pseudonyms (S-XXXX), the site name, age bands, year and month of the screen, and days since the participant's first screen. Also every measurement and the reference exam. Never: names, patient codes, file numbers, phone numbers or years of birth.

Getting access: a platform admin grants it in the admin console, for all clinics or one. Sign in with that email. Use the Site menu to look at one clinic at a time.

Tab What it shows
Overview Screens, participants, screens with a reference exam, inconclusive rate, median screen time; results and age bands; a table by site
Agreement Bland–Altman plots of OCIVO acuity against the reference exam (or the specialist's chart entry), overall and split into children (HOTV) and tumbling E. Shows mean difference and 95% limits of agreement
Accuracy OCIVO's referral decision against the examiner's judgement, with sensitivity, specificity, positive and negative predictive value and 95% confidence intervals. A second table covers eye-doctor referrals against non-glasses findings
Children Screens aged 3–11, the 3D check pass rate, children's accuracy, and the 3D check against amblyopia or strabismus
Camera bench Pooled camera bench results from all clinics, by distance
Export De-identified CSV, one row per screen, plus a data dictionary

The concept note's targets for agreement are a mean difference within ±0.10 logMAR and limits within ±0.20 logMAR. Inconclusive screens are left out of the accuracy tables and counted separately. Two eyes from one person are not independent, so report per-person results alongside.

Before enrolling study patients: confirm ethics approval at each site, and make sure the research consent wording is approved. Agree who does the reference exams and within how many days.

DoktorConnect connection

OCIVO is one of 14 engines on DoktorConnect Core. Linking a patient to their DoktorConnect ID (DC-ID) connects their vision record to the rest of their DoktorConnect care. Otivo, the hearing engine, links the same way.

Linking a patient. The DoktorConnect box is in the patient's history (Clinic tab) and in the patient's own My results.

What linking does

Area What happens
Blood pressure and sugar At the BP and sugar step, readings from the patient's DoktorConnect devices can be fetched instead of typed. Readings typed in OCIVO are saved to their DoktorConnect record
DoktorConnect app Core shows the latest vision screen and any open follow-ups in the member's summary
Messages Reminders, texts and emails go out through Core's shared messaging
Vision results Stay in OCIVO. Core shows a summary only; nothing is added to the vitals record

Messaging status (24 September 2026): email is on. Text messages start once the Africa's Talking (Nigeria) and Twilio (US) keys are added to Core. Numbers can be typed in any format, including 0803…, and still reach the right network.

Privacy and safety rules

OCIVO is built so that each person sees only what their role needs. The rules below are enforced by the database, not left to good behaviour.

Safety first

Privacy

Rule Why
Use a clinic file number, never a name, as the patient reference OCIVO does not need names to screen
A join code only sends a request; an admin must approve Nobody sees patients by knowing a code
Platform admins see counts, never patient records Onboarding does not need patient data
Researchers see de-identified, research-consented screens only Research consent is separate from screening consent
Saved screens cannot be changed The record stays trustworthy for care and research
Patient links are private, last 30 days and can be withdrawn Only the holder can open it; send it only to the patient or parent
Code slips and family invites carry no results A slip on a desk or a forwarded invite reveals nothing
A family label is a first name or a word like Mum The clinic sees it; no surname is needed
Camera pictures never leave the phone Only distances and a covered-eye yes or no are recorded
Messages carry no clinical detail beyond the next step A text seen by someone else reveals little

Before screening children at a school: collect written consent from a parent for each child, and agree in writing who keeps the forms and for how long.

Troubleshooting and quick reference

Most problems come from sign-in links, an unapproved account, or a missing setting on the server. The table below covers the common ones.

Problem What to do
The sign-in link says expired or already used Some email apps open links to scan them, which uses them up. Request a new link and open it in the same browser
"Waiting for approval" Ask your clinic admin to approve you in the Clinic tab or admin console, then sign in again
No admin access in /admin Only platform admins and clinic admins can use it. Ask a platform admin
No research access in /research Ask a platform admin to grant it in the admin console
The E or letters look the wrong size Redo the card calibration on that device. Keep full brightness and the exact distance
Many Test again results Check the room is well lit, the distance is right, the eye is fully covered and the patient understood the task
Camera chip says Camera off The browser blocked the camera, or the clinic has camera checks off. The screen still works without it
Voice answers not heard Allow the microphone. The site must show the padlock (https)
"Text: not sent" SMS keys are not set on DoktorConnect Core yet. Copy or share the link instead
"Email: not sent" The Resend key or sender domain on Core needs checking
A message mentions sql/00X That database update has not been run in the OCIVO Supabase project
The page looks old after an update Refresh; on a phone, close and reopen the tab

Check the system is up: open myocivo.com/.netlify/functions/health. It shows the build (currently ocivo 2026-09-24h), and every key should read true.

Quick reference

Item Value
Screen time target 8–10 minutes
Distance test 3 m (2 m small room); children 3–5 at 1.5 m
Near, macular grid, 3D check 40 cm, 30 cm, 40 cm
Calibration remembered 7 days per device
Patient code 6 characters, given by the clinic or in a family invite
Patient link Valid 30 days
Family members Up to 12 per household
Trend in My results 2 screens, 90 days apart; change of 0.15 logMAR
Reminders 08:00 UTC daily; day 3, day 10, weekly while overdue (up to 5), the day before a booked visit

All clinical thresholds in this guide are provisional until the consultant ophthalmologists sign them off.

OCIVO is a screening tool, not an eye examination. If anyone notices a sudden change in vision, a curtain or shadow, flashes with new floaters, or a painful red eye, they should get eye care today.
